Resumen

The OsteoMed ExtremiLock volar distal radius plate allows for fixation of distal radius fractures while preserving the pronator quadratus. Although disregarded by many, the pronator plays a role in distal radioulnar joint stability, as well as strength in forearm pronation. Surgical division or dissection of the pronator, which is necessary for the placement of other distal radius plates, interferes with postoperative forearm rotation. Pronator-sparing distal radius fixation allows patients immediate postoperative forearm rotation with full forearm rotation typically achieved within several days postoperatively. Furthermore, use of this technique facilitates early identification of those patients with distal radioulnar joint pathology and/or instability. Additionally, the pronator-sparing volar distal radius plate system is designed to facilitate optimal plate placement. The proximal location of the slotted hole combined with a patented centering clamp simplifies plate positioning. These unique features combined with cleats on the undersurface of the plate prevent eccentric placement onto the distal radius. The distal holes are directed further distal than most plates allowing it to sit proximal to the watershed line, preventing tendon injury and/or rupture. Furthermore, the pronator-spanning drill guide allows all holes to be drilled at one time, saving operative time. With screws offered in increments of 1 mm in length and the above attributes, the pronator-sparing volar distal radius plate makes distal radius fixation more efficient for the surgeon and shortens postoperative rehabilitation protocols for the patient with decreased risk of soft tissue injury.
